ELECTRONIC CONFIRMATION DEADLINE. Members will log in and confirm participation by each student. Print out the acceptance/invoice form. Completed Concert Festival Material to Bill Sittard. There will be a $100.00 Late Fee per school/sponsor assessed to those who do not meet the deadline. After 2/1/13, no forms will be accepted and student participation will be denied. Include the following: 1. Payment of Fees or School Purchase Order guaranteeing payment. 2. Acceptance-Invoice Form. 3. Health Forms. 4. Permission Forms. 5. Return unused music. There will be a $10.00 re-processing fee per every incomplete form received during this process.
